Their practices require enrollment. They are affiliated with MDVIP, the national leader in personalized and preventive care. Announcement Regarding Changes at CHFM

Dr. Mark Raupp will be leaving the practice of medicine effective 2/29/2012. All patients of Dr. Raupp will continue to be able to receive their health care at Chapel Hill Family Medicine in its current location over the next several months.

Chapel Hill Family Medicine is pleased to announce its planned association with the Duke Primary Care network. Within the next several months, the charts of all current patients of Dr. Mark Raupp and PA Joe Wilber will be transferred to the Duke Primary Care at Timberlyne office in Dawson Hall at the Vilcom Center on Weaver Dairy Road in Chapel Hill. At that time, patients will have the choice of continuing to be cared for by PA Wilber in the Timberlyne office or, alternatively, establishing care with one of the fine family physicians at Duke Primary Care Timberlyne.

If you choose to leave Chapel Hill Family Medicine and seek healthcare elsewhere, you can contact the local Medical Society at 919-833-3836, or contact your health insurance provider for a list of physician names and phone numbers. If you choose to select a new practice, you will need to complete an authorization for release of information so a copy of your medical records can be forwarded to your new physician. Your medical records cannot be released without a written authorization from you. These authorization requests are available at the Chapel Hill Family Medicine office located at 120 Conner Dr., Chapel Hill, NC; phone #919-967-8291 and fax #919-967-3627.

Dr. Patrick Guiteras and Dr. Jeffrey Furman will continue their MDVIP practices without changes at the current Chapel Hill Family Medicine location on Conner Drive. They are both accepting new patients and would be pleased to care for you and your family. If you would like further information about joining their MDVIP practices, or wish to set up an appointment to discuss their personalized and preventive medical practice model, please call the office at 967-8291.

Drs. Guiteras and Furman have been privileged to care for patients in this area over the last 35 years. It has been our honor and pleasure to deliver the highest quality care to you and your families. With changes in the current healthcare system, we feel an association with Duke Primary Care will allow Chapel Hill Family Medicine patients to continue to be cared for in a professional and top quality medical environment.

Procedure room for stress testing and surgical procedures This certification ensures that our physicians have been well trained in internal medicine, pediatrics, gynecology, surgery, and psychiatry and participate in continuing education activities to maintain current high standards of care. This diverse training makes it possible for us to provide the majority of health care you and your family need.

Services include health maintenance for well women and men, and well child care, geriatric care, dermatologic screening and treatment, patient education, personal counseling, and care of acute and chronic illness.
