If you are applying for
Canada Immigration you will need to bring the items listed below:
• Passport or copy of passport certified by a Canadian Visa Office
• List of current medications with dosage information
• Eye glasses or contact lenses, if you wear them
• Medical reports or test results that you have regarding to previous or existing medical conditions, and
• If you are being sponsored by a family member or spouse, Canada Immigration should have sent an IMM 1017E form. You will need to bring this. If you are applying through the Express Entry program, you will not have a form.

If you are applying for
Australian Immigration you will need to bring the items listed below:
• Your Referral Letter with your HAP ID
• List of current medications with dosage information
• Passport
• Your eyeglasses or contact lenses, if you wear them
• Any medical reports or test results regarding previous or existing medical conditions
If you have spent 28 days or longer in Afghanistan, Nigeria, Pakistan, Somalia, Kenya, Papua New Guinea, the Syrian Arab Republic, or the Democratic Republic of the Congo, or in any combination of these countries since May 5, 2014, you should bring your polio vaccination certificate to your medical appointment if you are applying from outside of Australia. Otherwise, Australia Immigration might request your certificate separately and this could delay your application.
If you are applying for
New Zealand Immigration you need to bring the items listed below:
• Passport
• Eyeglasses or contact lenses, if you wear them
• A list of any medication you are taking (including dosage)
• Any medical reports or test results that you have regarding to previous or existing medical conditions

General Medical Certificate Form
(complete form before medical exam)
or
•
Limited Medical Exam Certificate (complete form before medical exam)
or
•
Chest X-Ray Certificate (complete form before medical exam)
